All non-emergency Oregon Health Plan (OHP) transports that take place during business hours must be pre-approved and assigned by a caseworker with the Cascades East Ride Center before the transport can take place.

Ambulance Payment Plan

A minimum payment of $25 must be received by the monthly due date.  Invoice balance must be paid in full within one year from date of service.  Please contact Ofelia Escobar at 541-385-6682 to establish a payment plan.
